BOOKING CONDITIONS 

Prior to booking please ensure that you understand the following booking conditions: 

REBOOT Physiotherapy accepts:

  • Private paying patients (with or without private health insurance). 

  • Third party insurance cases, excluding NSW WorkCover. 

  • NDIS clients - self funded or plan managed. 

  • Allied / Extended Health Care Plans through Medicare - full payment is made at the time of your appointment and you then take your invoice to Medicare for reimbursement. 

COMCARE

Patients will be required to pay a gap payment of $45 at each appointment.

REBOOT Physiotherapy does not accept:

Department of Veterans Affairs (DVA). If you still wish to book you will be required to pay as a private patient.

Public Holidays 

REBOOT Physiotherapy will be

CLOSED on public holidays but may offer alternative hours on other week days during these weeks. 

COVID 19 Restrictions 

The COVID 19 Lockdown is now lifted in the ACT but some restrictions still remain and must be complied with. 

Patient are required to wear a mask at all times while in Health Space Weston and REBOOT Physiotherapy. CBR Check in and sanitising hands also required on entry. 

Please ensure that you meet the following criteria before booking:
 
DO NOT BOOK IF YOU OR HOUSEHOLD MEMBERS HAVE:
  1. Any symptoms of COVID-19 including but not limited to cough, sneeze, fever, sore throat. 

  2. Been to any exposure sites listed on the ACT Health website without completing the testing and isolation requirements. 

  3. Been in contact with known cases or those awaiting testing for COVID-19 or people who are unwell. 

  4. Not been adhering ACT Government Health directions.
